Sign Language Interpreter Request Form for Adults
This form is for Sign Language Interpreter Requests for immediate family members of students and employees of the Douglas County School District. Your request will be submitted directly to Anja Croon the ADA Manager, who will confirm your request and give you all additional information.
There is a 48-hour business day request policy. If possible, please do not wait until the 48-hour window of the event/ meeting to request an interpreter. A request is considered last minute, when placed within the 48-hour window prior to the date the service is needed. Interpreters cannot be guaranteed when a request is placed within 24-hours. Office hours (8:15 am – 4:30 pm, Monday - Friday). If Interpreters are needed for Monday morning meetings or events, the request needs to be submitted by Wednesday.
CANCELLATION / CHANGE OF MEETING OR EVENT
The ADA Manager needs to be notified immediately of any meeting/event changes (time, duration, location...etc) or cancellations for which an interpreter had been scheduled. There is also a 48-hour business day cancellation/ change policy. Therefore, all changes and cancellations are considered to be last minute, when placed in the 48-hour business day window. Cancellations within 48-hours of requested service cannot be made undone and require full payment, even without service provided. This also applies to changes in duration of meetings/events.
