Dealing with Asthma at School
If your student needs to use an asthma inhaler while at school, please have their healthcare provider fill out the linked Colorado Asthma Care Plan and Medication Order. This document provides the necessary medical orders and physician authorization. Additionally, please complete the linked Student Prescription and Over-The-Counter Medication Request and Release Agreement to give permission for school staff to assist with administering the medication.
To ensure School Nurse review and implementation of your child’s care plan and orders, please deliver the completed forms along with the pharmacy-labeled inhaler directly to the school’s main office.
