• Go to the PERA website at www.copera.org
• Select the 401K plan enrollment to request an enrollment form
• Select the 401K enrollment kit
• Complete the 401K contribution authorization form and send to the Payroll Department
• Once the funds have been deducted and sent to 401K
  (Citistreet), the funds will be put into a temporary account. 401K
  will notify you of your pin number
• Once you have received the pin number, you will need to go online and request the distribution of funds
