PLEASE NOTE: You do not need to redo any of the forms that you have previously received or entered into IC, prior to May 8, 2007. Simply begin using this new form instead of the old one from this date forward.

The top paragraphs regarding directory information have not changed. The body of the form has changed to allow greater understanding for granting or waiving permission for a student's photo (image) or schoolwork to be placed on the Internet, published by the district or by outside media (for example, newspapers, TV). The first section deals with Web/Internet Publishing. You will see that we separated student image from schoolwork so that parents can differentiate between the two. Parents should make only ONE choice under numbers 1 and 2.

Number 3, District publishing, refers to using a student's image, which may include photo, video and/or audio, for district purposes. This might include electronic presentations for use at a district conference,in the widely distributed annual report or displayed as wall art in the Administration Building. Again, parents should make one choice only.

Number 4 refers to allowing a student to be interviewed or photographed by external professional media representatives for use outside of the district, such as newspapers, TV or YourHub.com, an online media outlet sponsored by the Rocky Mountain News.

This form is good for the years that a student is in attendance at your school. Once the student moves on to another school, such as middle or high school, a new form must be completed by the parent or guardian.
